3. Consent

I hereby register bindingly for participation in the examination according to VdS 10013 Appendix B (DSMS Consultant). The examination is subject to a fee. The examination fee is determined according to the VdS fee schedule in effect at the time of registration, Module DSMB. It currently amounts to EUR 579.00 plus value-added tax. (Mandatory field) *
